For new students to CMC, is a form from the Internal Revenue Service. It is a request for the Name, Address and Social Security/Individual Taxpayer Identification Number for the student.

The W-9S form is related to the tax credit for the Hope Scholarship Credit and Lifetime Learning Credit The Hope Scholarship is for freshmen and sophomores who will pay tuition in 2004. The Lifetime Learning Credit is for Juniors, Seniors and Graduate students, and went into effect on July 1, 1998 for tuition paid after July 1, 1998. I know that some of you will not be eligible for these credits but we are still required by law to report the information. More information about the credits can be obtained from IRS Publication 970.

The Taxpayer Relief Act of 1997 now requires the College to report the Name, Address, and Social Security/Individual Taxpayer Identification Number for all students.

This form is required from all students every academic year even if your address has not changed.

Authorization of Allowable Charges Form PDF (33.3K)
This form is required from all new students even if you are not currently on Financial Aid.
